I would really like to transfer some of my forms to digital waivers in qualifications but I cant because of the limited fields to add. I need options for printed name and contact on the forms specifically for my records. (For example if I want to specify a parent's contact info on a waiver on a minors account)
I would also love if there was an option for Waivers to have a setting for staff (a program manager or site manager) to check off a field.

Plus one to be able to add name fields -- that is actually a minimum for legal language that applies to a waiver for a minor.
Also, we are struggling to figure out how to access the email of the parent/guardian who signed the waiver. We know the platform is collecting this information, but I just cannot figure out how to access it.
Program managers need to be able to send updates to parents/guardians, and it seems counterintuitive to collect this information twice.
I can see this data in the waiver certificates... but we can't click on each certification to extract the email,

Adding my two cents to the conversation. You and I have been chatting about this via email and wanted to share my thoughts with the others.
Agree that there needs to be a text field option for the waiver or consent document. Understand there can't be endless, but a few that could be added would be great.
We would like the name of the minor child on our consent form, which we can do with the fields you suggested above, but it puts it in a cursive font and treats it like a signature. We cannot have a minor child officially signing anything concerning liability or consent according to our state law. The text field(s) completed by a minor should allow the document to continue the waiver process of being forwarded to a parent or legal guardian for their signature. I even tried to do a workaround of using the initials field to capture the name of a minor child, but again, it is a cursive font and then when the minor child tries to submit for it to be routed to a parent or legal guardian, it has two pop-ups that ask them if they consent to e-signing the form. Our legal team was not pleased by that language and is preventing me from using your system to collect minor child consent forms this way. Now I'm forced to use the adult only waiver qualification and minor children cannot respond to our opportunities. This is an audience we are trying to reach and now we have this barrier. We will make it work in the meantime, but a few minor changes could be helpful in making the system work better for all.

I'll add this to the current card we have for this potential enhancement, but I did want to suggest adjusting the waiver to only need initials, signatures, and/or dates. Other information for your volunteers can be collected during User Registration by going to Settings > User Registration.

I wanted to say that all of points mentioned above would be beneficial to this update. Our waivers state what information we are looking for in the blank text area, so having a blank field option would be helpful.

Thanks to you both for adding thoughts to this thread!
Mary -- For basic profile items like Address and Phone Number, we recommend collecting this information in your user registration form (Settings > User Registration). Waivers only have fields available for signature, initial, and date, and these options and labels cannot be changed which could be confusing to your users. If you need more assistance on your registration form, please reach out to support in an email for help!
Mark -- This is a great question for us at support! I know it has been a couple weeks since this was originally posted. If you are still in need of help in regards to Adult/Minor waiver set up, please start a new ticket with us at support@galaxydigital.com. Additionally, this article gives more insight on how it looks for a minor then parent/guardian signs the waiver.

Yes, this would be excellent. One thing that we are grappling with is under-legal-age volunteers where the parent or guardian needs to sign. So the volunteers name would be the child but the parent would also need to add their name and signature. This brings in then the complexity of auditing, how would you know that it was the parent that signed? Signature routing?

Inserting custom fields into waivers would be incredibly useful for my site as well. I would love to have all of my clearances and waivers filled out electronically, but due to site limitations on what fields can be used in waivers, most of my forms must be printed and uploaded to the site. This not only is extra work for the volunteer and a barrier for volunteers without printers, but also is not environmentally friendly.
I cannot capture this information on other parts of the sites as the forms are from a county agency and must be submitted exactly as is for the them to run the clearances.

I'd love to add to this conversation if that's ok! I recently tried to upload a waiver that one of the cities we have volunteer events in requires for every single event. The volunteers have to provide a lot of info that's already in their profile like Name, Address, Phone Number, Emergency Contact, and I thought it would be so handy if there were fields we could add to the waiver forms like the signature block, but change the label so I could have them fill out the various fields of this waiver within the platform. I currently just have a blank one that they download and fill out, scan, and re-upload which works but is harder for some of the folks who don't have smartphones or scanners at home, or can't figure out how to edit the pdf.
Because this is an external form that this city provides, I can't work around it by asking for the information in different ways or through different channels on the site, as much as I wish I could.

Hi Shonie,
When I was talking about a "check-off" section I was thinking about something similar to how some forms have a "staff only" section where after the form is turned in. These sections are for staff to mark approval (which I know is a waiver option,) but also add information that a client would not know on their own or needs to be confirmed such as "staff visual confirmation of vaccine" or "days/areas approved for"
Essentially what I am getting at is I want to have some direct records of staff approvals not just inferred ones from the waver being approved or in a separate waiver in order to fully move my application process to GD. (Right now I am sort of stuck in a half and half area with online and paper)

Typically, we recommend using different areas on your site to collect additional information on a user within their profile, such as utilizing custom user registration questions to collect something like their parental contact information, for example.
